I tend to try scans after shutting down everything that's unnecessary in msconfig. If a hijacker tries to take over I'll revert to Safe Mode. I had one last week for cleaning that was the worst I've ever seen. Shut down all the startup stuff, rebooted to Safe and the cpu was still at 100% and mulitple attempts to start IE were running. Good thing I didn't have it connected to the web! Ended up pulling the hard drive on that one and slaving it to a workstation to clean it.
