I'd imagine it's more of a microfracture in the CD itself that was weakened because of the stress. Any object which is spinning has any point on it accelerating towards the center. So there's a fair amount of stress at any given point, and a microscopic irregularity could lead to higher local stress concentration, which increases the fracture, which increases the stress, until a part flies off, and I'd imagine the imbalance would accelerate the process from there.
It's the same reason you often see failures in things like hip implants, even when the overall stress is less than the yield stress, local stresses near an irregularity can be greater than the yield stress, causing fracturing and ultimately device failure.
