This is a screenshot of what i did:

excuse me if i've missed some steps but u get the general idea of it (am at work and harried on a monday).

btw, sometimes u can try dialling the whole thing in sequence :
like 18005551212,,,1,123,1-faxnumber

Also if u use the the aforementioned steps all u need to do is change the PIn when the card expires.
