We've managed to all but stamp out the Bubonic Plague, which is rare from what I know (which is not much in medical terms). I can't think of many deadly diseases off hand that were completely stamped out by humans, but that seems to be one.
Why do people call tech support for help with a problem, then lie about the problem itself? (can ya tell I'm at work now
