You can try going back into device manager and right clicking on the what is now listed as pci-modem, than go into properties and you may get a bit more info, I'm not sure on that one though but it's wortha shot. If not, open up the PC and visually inspect the modem and see if you can find out by looking at it, and to all your other questions the answer to every one is 'YES'...
