What kind of computer is this? Home built or name brand?

What are the complete system specs for this computer? List all parts by brand and model including the power supply.

Since the problem is still there after a format and re-install of Windows it looks like it's hardware based. Test the hard drive with the manufacturer's diagnostic utility.

Might want to change the data cable to the hard drive too.

And check that the power cable to the hard drive is inserted fully and securely.
