Refresh rate means the number of frames shown on your screen every second,
Refresh rate = 60hz
Number of frames every second = 60 frame/sec

The refresh rate of your monitor depends on your screen resolution. The higher the resolution, the lower the refresh rate.(mileage may vary ;p). A high quality monitor will support higher refresh rates at higher resolutions. If you want to avoid eye strain and headaches, try to run your monitor at 75 hz or better. You will notice flicker at lower refresh rates. That's what can cause eyestrain and headaches.

Only if you have a CRT choose the highest refresh rate possible. If you have a TFT, then run it at 60Hz, that's the best.

Isnt refresh rate the rate that pixels are energised on the phosphor screen of the monitor?
The frame rate can be locked to the refresh rate and will then run at that rate or less depending. Higher refresh is better as is higher frame rate.
You may have to fool with this setting to get the best result, ie; trying different ref settings and seeing if your frame rate drops in games due to the strain placed on the vidcard by a higher refrate.
I have seen that a higher refrate as in 85 and up, is far easier on the eyestrain created by viewing a monitor for hours.

If you disable V-Sync in your display properties, you won't be hindered by your refresh rate in your gaming.

V-Sync, to the best of my understanding, is the item that locks your refresh rate to your Frames Per Second. I may be off track on this, but I think if you disable it, you'll be able to send more frames per second than you refresh rate allows.

Thats the way I understand it at least.
Haveing the frame rate set to v synch will lock the rate at the refresh or less.
If you dont have v-synch turned on you will get more frames drawn by the card but there may be some visual stuff seen called tearing.
You will know it when you see it.

Yes v-sync does lock your fps to the refresh rate. Some games and or video cards request that you use v-sync as it will give a better picture. No tearing or clipping. When v-sync is turned off, your video card will paint as many frames as possible on the monitor. With so much overlap and supersaturation of the screen, tearing, clipping, and popping of the textures being displayed can occur. In other words your display will look smoother (sometimes) if v-sync is enabled. You have to try it both ways and see which gives you a better picture.
If it's pure fps you're looking for, by all means, disable v-sync.
