need some help pleas

specs

p4 3.2
2x 512
200gb HD
x700pro

it's been doind werd stuff and has only been getting worse, it's my card. It's doind this things; like stuff comes out the ground and everything is black, here i got pics to show.

im trying that MRA thing but WERE IS MY PRODUCT# AND MY SERIAL NUMBER? there are so many id's and number s and crap. I tryed some numbers but it always says im wrong.

Please make that a link, it makes the screen really wide - thanks.

I'm guessing the serial and model number will be on a sticker somewhere on the card.

thats guys i took the card out not knowing what the hell i was doing and toop pics and put it back in it was easy. Befor you get all OMG OMG, i used rubber gloves so i was grounded.
