Quake sound & video not in sync

When I play Quake, I boot up open Quake everything works well.
If for any reason I close Quake and later open it up the sound and video aren't in sync. If I reboot all is well.

This is how I open Quake on first boot. Click on icon, hit load, choose level, hit options and change video resolution to max.
Game plays normal.

If I close the game and reopen it change the resolution the sound and video are out of sync. There is about a 2 second delay.
If I don't change the resolution the games play normally.

Any ideas?

Delete and reinstall Q3 out of the question?
