video driver acting up

While playing a game it keeps crashing and error message says "nv4_disp driver stopped functioning normally" This is getting worse every time I try to play the game. What could be the problem? I've already reinstalled XP os and tried the driver from nVidia website to no avail. I did memtest86 to make sure RAM and CPU were not the cause, and passed the test. I did registry cleaner and spybot search & destroy as well as ad-aware to clean the system. So far nothing worked.

Any suggestions or ideas?

Does it happen on all games or just one?
What's the name of the game?
Are there any updates for the game you need to install?
