Connecting to Amplifier?

What kind of amp do i need to connect my pc sound card to it to have 5.1. My sound card has got 4 outputs in all. (Digital out), (Subwoofer, center), (Front left right), (Back left right). Im not gonna be using didgital out so count it out. I dont even know what it is. Can u have 5.1 going out on that 1 plug?

Anyway i got all the wires for all the 5.1 speaker. Center, subwoofer, front right, front left, back right, back left. That makes 6 wires in all for output to connect to my amp. But where exactly am i supposed to connect the wires. The amp is a 5.1 but it doesnt have 6 inputs like the sound card. How do i connect this.
