Graphics Card

I have a 512mb x1950 pro graphics card, when I went to install windows(which had problems) I got this blue error message. Now when I turn on my pc, I cant see anything on the screen, and I know its not the screen since it works with other pc's. Is it time to start RMA'ing it?

What kind of computer is this? Home built or name brand store bought?

What are the complete system specs? Please list the parts by brand and model including the power supply and the operating system.
