I recieved an e-mail from yahoo

I have an account on yahoo and i recieved an e-mail from yahoo which warned me about the removel of my yahoo account if i donot confirm my account on this e-mail then my account will be disabled or removed from yahoo and for that i have to fill in there small form available in the e-mail. I am suspicious about this e-mail and wanted to know that has any one of you recieved such e-mails?

Pls tell me what should i do because it is warning me that if i donot confirm my yahoo account within 2 weeks of the receipt of this e-mail, then my account will be permanently deleted.
