A firewall (such as kerio or sygate), and set your AIM security to allow only people on your buddy list to IM you. (preferences > privacy)

Oh wow. I use AIM but through Trillian - any suggestions there?

Same basic thing with trillian. Preferences > AIM > privacy. Under "who can contact me", select the option "allow only users on contact list".
