Document where user fills in info.

I need to make a document that has for example

Name: __________________ Last Name: ______________ Sex: __________

And so on. I want the person to be able to add the information but not change any of the fields that I have added. Under no circumstances should this person be able to change anything. I think I have seen pdf files like this but I need to know what program I can use to make it. I don’t want to make it in word or excel because this is for a moron that sometimes selects or deletes stuff.

Any help appreciated.

To make a fillable .pdf file, you'd need adobe acrobat (not the reader)...it's pretty expensive. I don't know of any free programs that would do it. Also, if you only have the reader, you can fill out the form, print it, but not save it.

You can create a form with Word as a template...it can be locked & password protected so the user can only fill in the blank fields. The easiest way would be to set it up in a table & use form fields via the forms toolbar. You can also specify in the field the max. no. of characters, etc.

If you're interested & need more info, do a search in Word help for forms or templates.

Word forms are pretty easy to set up, I'd set up the general form flow first and then go to Veiw- Toolbars- Forms and add the fields

I have the latest version of word and my choices of form fields includes:

Text Box - name address etc
Check box - pick one ie Sex - male female
Drop down list - Address State list etc
Lock form - stops people from making edits

If you need more math in the form you can use Excel instead -same path to get the toolbar.
