MS Access dialog boxes...

Hi,

The access form I've created at the moment is quite simple (this is the only Office program I don't have a clue about)

When I send somebody off or book them (I'm a football referee in the UK) I am required to send in a report form, I've made a simple access form and table so I can enter the details in and it will print out onto my form

I would like to make this even better by having a VBA set of dialog boxes open when I open the access file with "What was the player's name?" and the space to enter it, press Enter and "What was the incident" would come up etc

Any ideas?
