I haven't used ICQ for awhile but I think the newer version has a phone option. I know there are several programs available to do you you asked. Try a Google search.

pc to pc or pc to phone? there are lot of programs that make pc to pc for free but for pc to phone I have yet to find a free one.

Check out Skype, its freeware, but I think its only pc to pc capability.
