how to make a form in frontpage 2003?

I would like to make a form on my site like this site below

Where people can go type in information about themselves and it sends to me. I want the form to be done in Frontpage 2003, however I don't know advanced web design except for frontpage and HTML. I know a little php like in phpbb coding and stuff. What is the easiest way or is there some software (free/pay) that can help me acheive a form like the one on the site above?

I want, when people fill out the form it automaticly sends to my email (without the sender to use external email to send the form). Because most people I know use Yahoo or Hotmail web emails and do not use outlook express or external email programs. What is the easiest way in Microsoft Frontpage 2003? Thanks guys!
