Making a questionnaire website, video-answers, & proceed to next batch of questions

Hello.
I want to make a website that hosts few video files-which I made for my research purposes. I need the website to be in such a way that people cannot view the following question without answering current question. I also need to make sure that my video plays perfectly with its original resolution without any browser distortion. The video files are in standard WMV, MPEG2/4, AVI files. The answer sheet is an array of 10 pictures corresponding to the video which has been shown. The user can only choose 1 picture (by clicking on the picture/number below the picture) and confirm to proceed to the next question. I would also like to give freedom to the viewer to view/pause the video as much as he/she wants before moving on to the next question. The video will be 640x480.Basically there is no time limit to the video. The only thing which matters is everyone answers all the question. The only thing matters to me is that I get the answers and a brief questionnaire with optional fields.
I have a googlepages domain, and I have used the googlepages tool to construct a website and uploading it there. Its user friendly but limited on option. (That does not matter as I have never done any website based design aside from using googlepage creator so my knowledge is limited-hopefully I can master this new technique in 1 week with your help). Can anyone suggest a way for me to do the following website like I mentioned in the above text with a free WYSIWYG tool.
I have been told that googlepages does not support PHP and that I need to make my website using Java. So I need to write scripts? I think I can use the googlepages as the place where my video is right?
I also need a way to monitor the answers given by people who take my online questionaire.

* Have you considered embedding your video & your questionnaire into Flash? For example look at http://www.permadi.com/tutorial/flashMXQuiz/index.html
Instead of A, B, C you could have your video playing (you will need to go through some online tutorials or searches to embed video into a flash player.

* When you say "monitor the answers" if you mean get the answers, that becomes more challenging, because you usually need server side scripting to get the responses. Will flash in any way degrade my video quality? If not then I don't mind encoding it into flash. My videos only have moving images, and no audio, but the quality of the videos are important. I am measuring peoples recognition ability on stretched video resolution. Like spatial quantisation.
Something like the permadi link, but I don't want to reveal any answers to the test participants. I don't want those who have taken the test telling newcomers which are the right answers.

I just need feedback of people taking my test. It does not have to be fancy like where I can monitor live time. I can monitor it manually by each people placing their details after taking the test. I am testing a hypothesis, so I need feedback on my experiment thats all. Someone said that googlepages does not support PHP. If that is the case, I don't mind hosting my questionnaire somewhere else and only hotlink the video files from googlepages as it has 100mbs of storage.

In general, everytime you convert a video - especially high quality - into a more internet friendly format (like flash video) you should face degradation in quality. You could play around with it to see if it meets your satisfaction. Or you could just have the videos play in the format you want and have a separate section below the video for your survey. I suggested flash to overcome the lack of server side scripting on most free hosts.

Again, given googlepages not allowing any server side scripting your best bet would be using a survey in flash because javascript surveys are notoriously weak in providing any sort of integrity to your surveys (allowing the user to go back and forward etc etc). If you host your site elsewhere and then pull videos from googlepages, it gets really simple then.

I have followed your suggestion and did a flash video. It works quite well I must say since I made it 1600bit rate using riva flv encoder. The problem now is that I have done an into website, and a form website using http://www.phpform.org/. I got the questionnaire part running, but when I did try to make it stack in more than one column, it turns out weird without the background highlight option. Also, since I don't have any sort of webhosting, I have no clue if this work or not.
I tried reading Dreamweaver Cs3 book for dummies on how to make radio buttons and tick boxes, only that I got lost, and to the point of I could not care anymore. The problem might seem small is that even if I manage to make the website with the script functioning (if I ever get the script to work in the first place), I need to host it online (which I don't have any experience at all).
I am contemplating on using survey websites that can let me upload my questions there. The only problem i see is that with survey websites, most do not allow me to upload files, or like I need to subscribe in order to upload files to the space. Also, I am not too sure if it will go well as the survey has slightly sensitive material which I don't want random public to get access. A survey with a password to do the survey will work well.
Really right now I am more or less confused on what to do. Should I finish on doing the survey website from ground up, or just go with those online survey materials.

Have you considered paying a nominal fee for a full-featured webhost? I'm certain you can find one for less than $10 USD a month.

Yes, that is something I have been exploring. If I get a full-featured webhost, I still need to make the php script which I actually have no idea on how to write any codes. I know how to make radio buttons and tick boxes, I am just not sure on how to get feedback from the questions posted.
http://questionform.com/ looks like a good simple solution but I am opened to any suggestions which can prevent any sort of unnecessary headache. Problem is ready made templates may be a bit too rigid for me. So far I have managed to read through the Dw CS3 for dummies book without problems aside from the buttons part. I know that I need to get feedback, but I am not sure how I go about it. I still need to find a method of restricting outsiders from accessing the website.A simple password for site entry is needed too.
