I have a small web survey, how do I get the result and post on the web?

I created a small web survey, there is only one question.

The question is how do I improve the website?
A. need better content
B. need better design layout
c. need better nav bar

when the user click submit
I would like to know do I need to make a web database and calculate the total number? what software do I need to use?

One simple option is just to use javascript (attached) for the end users to submit their opinion. It won't be secure, but probably doesn't matter in this case. It won't have advanced features like calculating submissions, etc... but should suite your need. Again this is just one option and probably the easiest.
