Scan with your antivirus (and probably an online scanner), antispyware apps to see if anything is lurking somewhere causing this to happen - also check your start up applications (start, run, services.msc and msconfig) and if you have anything special relating to the mouse - like a driver or customisation program, then disable it and see what happens.

Yes I did a Spybot and a Microsoft nothing turned up. I also tried a system restore and that did not work.

Like whatsthisboxfor? said do you have speical drivers installed for your mouse? I'm thinking something weird happened with your mouse driver that makes it unresponsive and that it works in safe mode because its loading a default mouse driver.

No I don't have special drivers. All works well in safe mode that's what funny.

Another detail I noticed I can use the keyboard to get around but the mouse is very limited, it can only right click and that's about it.

Just a hunch, but worth a try. Go in Device Manager and uninstall your mouse. Then reboot and let XP (assuming XP) find it again and reasign the driver.
