Your question is a bit unclear.

Do you want to add and remove the start menu or "items" on the start menu?

To decide which items run at start up enter msconfig
go to run and type ''msconfig'' without the quotation marks.
Then go to start up and select which you want to keep and which you don't.

Ahh I understand now - With my XP I can right click on the start menu item and select unpin from start menu.....This is not a function that I have seen on all XPs - mine at school don't allow it.

On the all programs menu you can drag and drop the shortcut for a program you want to run at startup into the startup folder. Same thing in reverse to get them out of there, just drag them and drop them wherever you want them in the list.
