blaster and nachi

when a customer calls with symptoms that sound like blaster i normally turned off system restore and ran stinger then turned on system restore and applied patch KB823980-X86. NOW i notice on the web that patch should be run before removal of virus/worm.
Question is this. Does the KB82824146-x86 patch replace the older blaster patch. Can it be used instead or is it for entirely different problem?
The nachi thing supposedly deletes the blaster and applies some microsoft patch/s. What is the best way to tackle this problem in the field where you don't really know what your are dealing with (blaster or nachi)
thanks,
alan
