ltmdmnt.sys = Lucent Modem driver
Did you install the driver that came with the modem or did you use the driver that XP has in it.

I installed the driver from the ActionTec CD that came with the hardware box , and it's a ActionTec driver, as seen in DM. Maybe they are using lucent drivers for it

Do a google search for ltmdmnt.sys and see if any of the fixes will work or uninstall the modem and let windows use it's built in driver.

I've been reading the booklet for on the driver installation and they seem to suggest that, one install\reinstall they driver for this modem first, and reboot, so that win does not install it's own driver for it, I don't know why or it may just be 'cause they have a newer version then win, this modem it's been a sweetheart it's fast reliable (till now) it connects first shot seamlessly I have had a US Robotic modem and I had nothing but problem with.....So we'll see

Have you tried to go to the device manager and then do an update driver, pointing it to the cd. It is possible that the windows driver became corrupted. Or one of the other things is to move the modem to another PCI slot, just to rule out IRQ problems.
