Files that could not be defragmented

I just got done defragging my HDD using Disk Defragmenter, but the report showed that many files could not be defragmented (around 20 files). Is there something I can do about these files?
I'm not sure how exactly files get fragmented, but I noticed that most of these files have been moved from one folder to another, could this be it? And is there any way I could avoid fragmenting files by maybe keeping them in one folder rather than moving them around?

Try defragging while in safe mode.

Exactly, moving them around will fragment them.

Using your computer like you normally would, you're going to get fragmented files no matter what you do...that's just how Windows is. The only way you can keep the files on your computer from becoming fragmented is to leave it off.

Just run defrag on a regular basis to keep the fragmentation down to a minimum.
